Service Description

Comprehensive 2-hour session including home and client safety assessment with a written report on recommendations and needed adaptions. Detailed advising and problem solving for barriers to daily independence in self care, medication management, meals, transportation, exercise, community and socialization. We also provide referrals to home care aids, therapy, legal and financial referrals, transportation services, and more. Gold service includes a follow up phone call to review and answer questions.
